| An employee was troubleshooting an aircraft from the cockpit. The employee exited the left side of the cockpit and stepped on the engine intake. When his weight shifted to his right foot, his foot slipped off the intake and he fell off the aircraft. | An employee was using a crowbar to lift the back left side of a section of electrical switchgear cabinet to install a shim. The crowbar rolled, and the weight of the gear came down onto the tip of the flat piece of metal he was pushing the shim with. His left index finger was pinched between the shim metal and the concrete pad amputating the fingertip. | |||||
